Intake forms

Collect client information and agreements before their appointment.

Intake forms collect what you need from a client before they're in your chair — health questions, sensitivities, consent acknowledgments.

Set up your form

  1. On the dashboard, go to Settings → Intake Form.
  2. Build the questions you want every new client to answer, and enable the form.
  3. Use the preview to see exactly what clients will see.

How clients get it

  • Automatically — with the intake-form notification enabled (Settings → Notifications), clients are sent the form around their booking.
  • Manually — from a client's record you can send (or re-send) the form link whenever you like.

Clients fill the form from the link on their phone — no account or app needed.

Where responses go

Completed forms attach to the client's record, so their answers are on hand at the appointment. Check them before the client arrives — that's where the "allergic to latex" answer lives.

Tips

  • Keep it short. Every extra question costs completion rate.
  • Anything critical from a response (like an allergy) is worth copying into the client's notes so it's impossible to miss later.
